With an HMO D-SNP, you may be eligible for an Anthem Benefits Prepaid Card with a monthly or quarterly allowance that you can use to pay for everyday items and expenses, such as:

Our PPO D-SNP plans do not include groceries or utilities and only some include an OTC benefit.

When Can I Enroll In A D-SNP?

If you have Medicare Part A and Part B, you can enroll in a Dual Special Needs Plan the month you become eligible for Medi-Cal.

 

If you are already enrolled in a D-SNP plan, you may be able to change plans monthly depending on your current plan and/or the plan you want to enroll in. And if you’ve had a life event such as moving, losing coverage or newly eligible for Medicare, you may qualify for a Special Enrollment Period. You can always change plans during the Annual Enrollment Period from October 15 to December 7 every year.

Frequently Asked Questions About D-SNPs

Dual eligible means that you qualify both for Original Medicare as well as Medi-Cal. The state of California determines who qualifies for Medi-Cal and what types of benefits you can receive. 

Original Medicare is coverage managed by the federal government for people 65 or older, younger people with certain disabilities, and people with End-Stage Renal disease. Medi-Cal is a state program that provides medical and/or supplementary benefits based on your income level. 

Many D-SNPs come with $0 premiums, copays, and deductibles4. If you receive full Medi-Cal benefits, nearly all of your out-of-pocket costs should be covered.

Full dual eligible means you receive full Medi-Cal benefits. Your Medicare Part A and B premiums and nearly all of your out-of-pocket costs may be paid based on your state's program. 

No, not at all. A D-SNP lets you keep all your Medi-Cal benefits and coordinates them with your Medicare coverage. That way you can access all your benefits more easily. With an Anthem D-SNP, you'll get valuable benefits and the support you need.

 

But it's important to keep in mind that you need to renew your Medicaid coverage every year to stay enrolled in a D-SNP. Medicaid benefits are reviewed by the state agency to make sure you're still eligible. You will get a notification from your state agency when it's time to complete your renewal. Make sure to return it right away because if your Medicaid coverage isn't renewed, you may lose eligibility for a D-SNP as you need both Medicaid and Medicare. 1 Members may receive a monthly or quarterly allowance in the form of a benefits prepaid card to pay for a wide range of approved groceries and utilities. Unused amounts will expire at the end of the month or quarter. Benefits vary by plan.

 

2 Depending on your plan, the benefits listed may be available through the Medicare Advantage Value-Based Insurance Design Model or to eligible members as Special Supplemental Benefits for the Chronically Ill (SSBCI). You may qualify for SSBCI if you have a high risk for hospitalization and require intensive care coordination to manage chronic conditions such as Chronic Kidney Diseases, Chronic Lung Disorders, Cardiovascular Disorders, Chronic Heart Failure, or Diabetes.
