Medicare Advantage SNPs are for people with disabilities, living with certain medical conditions, or those who qualify for Medicare and Medi-Cal. These plans include Original Medicare Part A (hospital) and Part B (medical) and add many valuable benefits.

Dual Special Needs Plans (D-SNPs)

Anthem has full and partial dual eligible plans. If you’re receiving full medical benefits from Medi-Cal, almost all of your out-of-pocket costs may be paid. Plan availability, premiums, benefits, and benefit amounts may vary based on where you live, the plan you choose, and your Medi-Cal eligibility status. Our PPO D-SNP plans do not include groceries or utilities and only some include an OTC benefit.

Institutional Special Needs Plans (I-SNPs)

 

Anthem offers Institutional Special Needs Plans (I-SNPs) in California. You are eligible for an I-SNP if you expect to be in a long-term care (LTC) or skilled nursing facility, or if you need intermediate care for 90 days or longer.

Am I Eligible And How Do I Enroll?

 

You can enroll in an Anthem Special Needs Plan (SNP) if you meet certain qualifications by plan type:


 

D-SNPs 


Plans require that you are eligible for both Medicare and Medi-Cal.

Full dual eligible means that you also qualify for full-state Medi-Cal benefits.

If you have Medicare Part A and Part B, you can enroll in a Dual Special Needs Plan the month you become fully eligible for Medi-Cal.

 

If you are already enrolled in a D-SNP plan, you may be able to change plans monthly depending on your current plan and/or the plan you want to enroll in. And if you've had a life event such as moving, losing coverage or newly eligible for Medicare, you may qualify for a Special Enrollment Period. You can always change plans during the Annual Enrollment Period from October 15 to December 7 every year.

   


 

C-SNPs 


Plans require that your doctor certifies that you have one of the following chronic conditions: diabetes, a heart condition, lung disease, or end-stage renal disease.

You can enroll when you’re first diagnosed with a qualifying chronic condition or during the Medicare Annual Enrollment Period (AEP). You may also qualify for a Special Enrollment Period (SEP).

This list does not include all the benefits that come with Special Needs Plans. Plan availability, premiums, benefits, and benefit amounts may vary based on where you live, the plan you choose, and your eligibility.

Many SNPs are available with $0 premiums, deductibles, and copays. Depending on eligibility, some plans may charge a monthly premium in addition to the Part B premium. Deductibles, copayments, and other costs may vary based on the plan you choose and where you live. If you have Medicare and Medi-Cal, most of the costs may be covered for you by the Medi-Cal program.

Specific groups of people can qualify for one of three types of SNPs:

 

    Dual Special Needs Plans (D-SNPs) for people who are eligible for both Medicare and Medi-Cal

 

    Chronic Special Needs Plans (C-SNPs) for people with diabetes, heart, lung or kidney disease

 

    Institutional Special Needs Plans (I-SNPs) for residents of long-term care facilities

1 Members may receive a monthly or quarterly allowance in the form of a benefits prepaid card to pay for a wide range of approved groceries and utilities. Unused amounts will expire at the end of the month or quarter. Benefits vary by plan.

 

2 Depending on your plan, the benefits listed may be available through the Medicare Advantage Value-Based Insurance Design Model or to eligible members as Special Supplemental Benefits for the Chronically Ill (SSBCI). You may qualify for SSBCI if you have a high risk for hospitalization and require intensive care coordination to manage chronic conditions such as Chronic Kidney Diseases, Chronic Lung Disorders, Cardiovascular Disorders, Chronic Heart Failure, or Diabetes.
