Are You Eligible For A C-SNP?
C-SNPs are designed to meet the unique needs of people living with diabetes, heart disease, lung conditions, and end-stage renal disease (ESRD). Anthem offers three types of C-SNPs with benefits tailored to specific needs:
Chronic Care
Chronic Care C-SNPs offer support and care for people living with:
- Diabetes
- Chronic heart failure (CHF)
- Cardiovascular disorders
Most Chronic Care C-SNPs include:
- Blood glucose monitoring
- Nutritional education and training
- Insulin management
- A diabetes self-management program
- Foot and wound care
- A SilverSneakers® fitness program
- A dedicated care team of an endocrinologist (diabetes specialist) and/or a cardiologist (heart specialist)
Lung Care
Lung Care C-SNPs offer support and care for people living with:
- Chronic obstructive pulmonary disease (COPD)
- Asthma
- Chronic bronchitis
- Pulmonary hypertension
- Emphysema
Most Lung Care C-SNPs include:
- Special coverage for oxygen supplies and other medical equipment
- A SilverSneakers® fitness program
- Care from a primary doctor and a pulmonologist (lung specialist)
- A dedicated care support team
Kidney Care
Kidney Care C-SNPs offer support and care for people living with end-stage renal disease requiring any mode of dialysis.
Most Kidney Care C-SNPs include:
- Transportation to and from dialysis appointments1
- Education, self-training, services, and supplies to support dialysis and manage care
- Allowance for over-the-counter (OTC) health items
- Care from a primary doctor and a nephrologist (kidney doctor)
- Nutritional foods tailored to your chronic condition

Is Another Medicare Advantage Plan Right For You?
If you’re not eligible for a Chronic Condition Special Needs Plan, Anthem has other options. If you have both Medicare and Medicaid, you may be eligible for a Dual Special Needs Plan (D-SNP). Or consider a Medicare Advantage plan that has doctor, hospital, and prescription drug coverage all in one plan.
